Description: Develops, plans and implements measurable goals, objectives and activities for persons served as identified in their Individual Service Plan. Will develop assessment summaries, write goals and objectives, prepare and participate in staff meetings, administer medications, treatments and case note/document as required for persons served. The planner will assist in the delivery of vocational and community integrated activities and events. This person will ensure quality, person-focused services are provided to persons served, families, customers, vendors and staff. Supervision of direct report staff to include monitoring time and activities, scheduling, training, performance evaluations, and staff development. Works closely to coordinate production work in the DAC. Will receive and report accidents/seizure reports, abuse/neglect, and complete emergency control procedures as required. Will provide direct personal care to persons served as needed, including medication administration, toileting, etc.

Qualifications: Bachelors degree in Human Services field, and one year experience with developmentally disabled or elderly. ABILITY to lift 50 lbs, and will pass a background check and MVR or offer of employment to be rescinded.
